What if the scan shows a problem? What is an ultrasound scan? An ultrasound scan sends sound waves through your womb uterus. These waves bounce off your baby as echoes. Hard tissues, such as bone, reflect the most sound waves and so make the biggest echoes. These appear white in the image, and soft tissues appear grey. Fluids, such as the amniotic fluid surrounding your baby, appear black. This is because the sound waves go through them with no echoes.
Use LMP or 12 week scan date
Pre-scan preparation Need to expose abdomen so ideally wear 2 piece garments i. Please do not empty your bladder before the examination. Guide to number of guests Most clinics can comfortably accommodate 5 guests including children for this scan.
If they have concerns, they can discuss it with you in an objective and non-judgmental way.
Measuring[ edit ] Diagram showing crown-rump length by gestational age. The blue line is the mean and the green area delimits the 3rd versus the 97th percentile. The measurement can actually vary slightly if the fetus is temporarily stretching straightening its body. The measurement needs to be in the natural state with an unstretched body which is actually C shaped. The measurement of CRL is useful in determining the gestational age menstrual age starting from the first day of the last menstrual period and thus the expected date of delivery EDD.
Different babies do grow at different rates and thus the gestational age is an approximation. Recent evidence has indicated that CRL growth and thus the approximation of gestational age may be influenced by maternal factors such as age, smoking , and folic acid intake. In that situation, other parameters can be used in addition to CRL. The length of the umbilical cord is approximately equal to the CRL throughout pregnancy.
Gestational age is not the same as fertilization age. It takes about 14 days from the first day of the last menstrual period for conception to take place and thus for the conceptus to form. The age from this point in time conception is called the fertilization age and is thus 2 weeks shorter than the gestational age.
Thus a 6-week gestational age would be a 4-week fertilization age.
We validated the performance of our chart using an independent twin cohort, and showed in a further cohort of consecutive ultrasounds it would materially alter dates if it used instead of any of six preexisting charts. Hence we believe our chart may possibly be the most accurate of any published chart to date pregnancies between 6—9 weeks of gestation. In addition, we found inaccuracies in the ability of commonly used charts to estimate gestational age among IVF singleton and twin cohorts where exact dates are known.
These recommendations apply to routine screening ultrasonography and not to diagnostic ultrasonography for specific clinical indications e.
Dating and viability What is a dating scan? A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Dating scans also reveal other important information such as: Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures.
For transvaginal ultrasound you can have an empty bladder When is it recommended? Dating scans are usually recommended if there is doubt about the validity of the last menstrual period. For example if you do not know when the first day of your last period, or the likely day of conception was have had irregular periods have had an unusual cycle because you recently stopped the oral contraceptive pill, or you recently had a miscarriage, or you are breast-feeding had some implantation bleeding which can be been confused with your last period date What can be seen?
By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures.
How accurate are ultrasounds
However, to be effective it is recommended that scanning services follow standard protocols based on national guidelines and that scanning practice is audited to ensure consistency. Bradford has a multi-ethnic population with one of the highest rates of birth defects in the UK and it requires an effective foetal anomaly screening service.
We implemented a rolling programme of audits of dating scans, foetal anomaly scans and growth scans carried out by sonographers in Bradford. All three categories of scan were audited using measurable parameters based on national guidelines. Following feedback and re-training to address issues identified, re-audits of dating and foetal anomaly scans were carried out. In both cases, sonographers being re-audited had a marked improvement in their practice.

Heart The heart can be recognized as a beating, large and bright structure below the embryonic head at 7 weeks. The heart rate increases from bpm to bpm. Details of the heart anatomy are not visible, but the atrial and ventricular compartments can sometimes be distinguished by the reciprocal movements of the walls. Intestinal tract The short umbilical cord shows a large celomic cavity at its insertion, where the primary intestinal loop can be identified. The first sign of herniation of the gut occurs during week 7 as a thickening of the cord and showing a slight echogenic area at the abdominal insertion.
Within a few days, this echogenic structure becomes more distinct. Extra-embryonic structures The amniotic cavity becomes visible at the beginning of week 7. The mean diameter of the amniotic cavity is almost the same as the corresponding crown—rump length. Identification of the yolk sac and the placenta vascularization and yolk sac 8 weeks crown—rump length 15—22 mm Gestational Sac and Yolk Sac External form The body gradually grows thicker and becomes cuboidal.
At the end of the week, the elbows become obvious, the hands angle from the sagittal plane and the fingers are distinguishable. Figure 4 – 3D scan – the body grows, visualization the future upper and lower limbs. The hemispheres enlarge, developing via thick round slices originating antero-caudally from the third ventricle into a crescent shape. The choroid plexus in the lateral ventricles becomes visible as tiny echogenic areas. The future foramina of Monro become more accentuated during week 8.

Of all of these uses, dating the pregnancy is the most common reason to use ultrasound, particularly when the expectant mother cannot remember the date of her last period as in breast-feeding or irregular cycles.
When an ultrasound is performed, measurements of the head, abdomen, thigh, and amount of amniotic fluid are done.

Abstract Objective s We intend to verify if fetal volume and crown-rump length were different between singletons and twins in pregnancies aged from 7 to 10 weeks and to evaluate if fetal volume is more accurate to determine the gestational age than crown-rump length at this gestational age. Study design From 52 days 7 weeks and 3 days to 73 days 10 weeks and 3 days weekly three-dimensional ultrasonography was performed in 20 twin fetuses and 20 singletons.
Results At the age of 52 days, the crown-rump length was At the gestational age of 73 days, the crown-rump length was The total relative increase observed was much higher for fetal volume than for CRL: No significant difference was identified between singletons and twins for both fetal volume and crown-rump length. Conclusion s Twins and singletons had similar fetal volume and crown-rump length between the 7th and 10th week of gestational age. Additionally, fetal volume assessed by VOCAL was better than crown-rump length to estimate the gestational age at the evaluated period.
However, the improvement was small and probably without clinical significance. Condensation Fetal volume and crown-rump length were similar between singletons and twins. Fetal volume relative increase was higher and the predicted gestational age was better. Previous article in issue.

References “The technology of prenatal diagnosis is usually presented to us as a solution, but it brings with it problems of its own If you’ve already read it, you may want to skip ahead. All pregnant women in our technology-happy modern society face confusing choices about prenatal testing, its advantages and disadvantages, and its appropriateness for them.
Clearer images are obtained because the probe is of higher frequency and is nearer to the object of interest in the womb.

Hutchon’s site and the Gestation Network also provide pregnancy calculators. A large number of mails I received are about fetal anomalies, I have therefore tried to put together a Catalogue of Web pages which describe in some detail specific congenital anomalies that are diagnosable by ultrasound. You can visit the page here. I am pregnant for 7 weeks. I went for a scan yesterday and was told I should have a full bladder.
They made me drink 4 cups of water and my bladder was almost bursting at the end of the session. Is this really necessary? A full bladder is usually necessary when the scan is done abdominally and in the early part of pregnancy. This would tend to lift the uterus up a little bit and being in front displaces the bowels away and act as an echo-lucent windows for the ultrasound to pass through. Ultrasound doesn’t go through air as in the bowels well but transmits perfectly through liquid as in the case of the urine in the bladder.

It was a long wait to say the least. Another name for the 20 week ultrasound is a foetal anomaly or foetal morphology scan and it can be done anytime between weeks of gestation.
Your first scan won’t be able to tell you the sex of your baby just yet.
Ultrasonic Fetal Measurement Standards If we assume a fetus is growing normally, biometric measurements are determined by gestational age, and we can estimate the gestational age and thus the due date. Conditions which alter fetal growth will make the estimates less reliable. Although many embryonic and fetal structures can be measured, only a few measurements are easy and repeatable enough for widespread use.
The most common are: The first element to be measurable is the gestation sac of the early pregnancy. The gestational sac is measured in three dimensions, and the average, the Mean Sac Diameter MSD used for estimating gestational age. The length of the embryo on the longest axis excluding the yolk sac constitutes the crown-rump length.
The transverse width of the head at it’s widest. We measure from the the leading edge to leading edge of the bones, because this leading interface is most distinct. Since the head is oval, the error induced by small errors in positioning is small, making for a repeatable, robust measure. The BPD best used after 12 weeks.
6 Weeks Pregnant
First trimester ultrasound is performed in the first months of a pregnancy. Pregnancy ultrasounds are performed mainly using transabdominal ultrasound. For many women, especially after 8 weeks gestation, sufficient information about the baby may be obtained with transabdominal ultrasound only.
It would also offer the possibility of operating at higher fields to affect a potential reduction in the GIS size with subsequent savings in the cost of manufacture and installation.
This is generally the earliest that anything is visible on an ultrasound, although at this point it will not look much like a baby, since development is still in the early stages. Video of the Day Ultrasound Type At this early in a pregnancy, it is difficult to see anything at all using a standard external ultrasound machine. Often, if a six-week sonogram is performed at all, it will be using a vaginal ultrasound machine, which consists of a wand-shaped device that is inserted into the vagina.
Location A six-week ultrasound can determine the location of the embryo and ascertain that it is in the correct place in the uterus. If the pregnancy is an ectopic pregnancy, with the embryo implanted outside the uterus in the Fallopian tube, this can be determined based on the blood flow patterns seen via ultrasound. Heartbeat The fetal heartbeat is often detectable at a six-week ultrasound. The normal fetal heartbeat at six weeks is about 90 to beats per minute.
Detecting a heartbeat at this stage indicates that the pregnancy will probably continue and not end in miscarriage, although this is not absolutely guaranteed.
The week scan
If any woman suspects that she has had a miscarriage, then ultrasound is the most common test that she will undergo. This test can also be recommended by her caregiver if he or she suspects the same. This test usually goes along with hCG blood test. Another common term that is given to the ultrasound is sonograms.
I had a scan at 8 weeks and my doctor said the fetus is only 7 weeks, and it was because I had irregular menstrual cycles.
Can the scan show the gender of my baby? What is actually done? Scans are usually performed by a doctors, midwives or radiographers who are specially trained in ultrasound, and are known as sonographers. The whole pregnancy will be assessed. The amount of liquor amniotic fluid , the position and appearance of the placenta and a detailed scan of the baby itself. The baby will be measured, and the anatomy examined in detail.
