A teenager had been the subject of internet images in which she was sexually assaulted. She and her brother had been ill-treated by their mother and sexually abused by their uncle. Her injuries resembled those from a car crash. A serious case review made 90 recommendations and highlighted a string of mistakes by 11 agencies.
